Employees with Concerns

For employee questions about or requests for disability related accommodations and/or complaints of alleged discrimination, including sexual harassment, contact hreeoc@9416hd44.com.

Staff members can also use the SPS Discrimination Complaint Form to submit a complaint in writing. The use of a particular form is optional and not required. Employees should submit the written complaint to the following:

Filing a Complaint

Students, parents and members of the public can submit a formal complaint, which must be in writing. The use of the SPS Discrimination Form can assist in this process:  SPS Discrimination Complaint Form

Instructions

  1. Open the SPS Discrimination Complaint Form
  2. Please complete all sections of the form. Please note that the use of a specific form is not required; you can email a school leader or the Office of Student Civil Rights (oscr@9416hd44.com) directly with the information below.
  3. Be as specific as possible when describing the incident(s). Your complaint is not limited to the space provided. You are encouraged to attach additional materials, which may assist in the investigation process.
  4. Please be sure to include the following information either on the form or in an email:
    • the date(s),
    • the description of the incident(s) that occurred,
    • the name(s) of the person(s) involved, and
    • the name(s) of those who may have witnessed the incident(s).
  5. Deliver/send/email the form or email to either the Office of Student Civil Rights or any school principal.
